Description

Under the supervision of an ICHD Supervisor and in partnership with the Advance Peace Strategy Manager responsible for administering the Peacemaker Fellowship, assists Advance Peace Neighborhood Change Agents (NCAs) and Fellows with carrying out the functions of specific health programs such as Medicaid Outreach services, Medicaid-outreach related mobile health unit programming, Substance Use Disorder (SUD) navigation support services (with emphasis on Medicaid outreach), SUD and treatment services (with emphasis on Medicaid outreach), Post Traumatic Stress Disorder (PTSD) recovery services, group counseling services and access to other individual and group Cognitive Behavioral Health (CBT) services. Also supports Fellows and NCAs in navigating community resources to access and meet needs connected to social determinants of health (SDoH).
Duties may include participating in case conferencing related to Fellows, providing Medicaid outreach services, coordinating group and individual PTSD and CBT therapy sessions, locating and disseminating resources related to SDoH, attending coalition and partnership meetings, ensuring year-end budget spend-down, and participating in other individual / direct service provision with Fellows and supporting NCAs in ensuring Fellows are able to connect to community resources and supports.
May also enroll Fellows in health insurance plans and other programs and answer questions related to community resource procedures, eligibility requirements, scheduling, and related matters.
Essential Functions
1. Participates in specific health department programs by assisting with the planning, coordination, and implementation of programs, coalition meetings and events.
2. Coordinates group CBT and/or PTSD recovery sessions and other events to ensure Fellow are able to healthcare and other services and resources related to social determinants of health across agency providers and systems.
3. Assists coalitions formed to work with specific Health Department and Advance Peace programs. May assist with planning and scheduling meetings, facilitating meetings, assisting with policy development and recruiting appropriate persons to be coalition members.
4. Maintains records and statistics on program activities and prepares activity and financial reports as required by grant funding sources. May assist with preparation of grant funding requests.
5. Markets and interprets the goals and objectives of community violence prevention programs to community groups, schools, professionals, and other interested people. Promotes and conducts local activities that are part of statewide campaigns.
6. Participates in staff meetings and attends and/or conducts conferences and workshops. Represents the Health Department on various councils and task forces.
7. Coordinates services and networks with schools, businesses, community organizations, and other agencies in identifying community needs and providing information or programming.
8. Answers questions in person and by telephone regarding departmental procedures and requirements, program requirements, scheduling information, and other activities. Schedules clients for various programs, appointments, group sessions and/or clinics. Assists clients in filling out forms and reviews documents for proper completion.
9. Monitors client health plan applications and ensures that proper documents are completed.
10. Prepares, sorts, processes and files a variety of forms, applications, documents and records in accordance with departmental procedures and program guidelines.
11. Provides outreach and education to clients and medical staff regarding program guidelines and health department services through mailings, displays, telephone contact and in person.
12. Coordinates and facilitates community partner meetings as needed.

*May monitor program data, create documents and forms, and work with other health departments to coordinate program activities.

Employment Qualifications
Education: A minimum of a High school graduation or equivalent is required.
Experience: A minimum of Two years of job-related experience such as health care, navigation, community services outreach, substance use disorder support services and/or working with justice-involved individuals is required. May require experience and training in various computer software and equipment.

May require the ability to climb stairs to access work sites.
Working Conditions:
Works in office and clinic conditions.
May work in various off-site locations throughout the county.
May be exposed to communicable diseases, blood, and other bodily fluids.
