ESSENTIAL FUNCTIONS
Establishes good working rapport with referral sources. Obtains all information necessary to make admission decision including patient data, insurance information and medical diagnosis.
Works closely with outreach liasons to facilitate onsite visits to meet with family members and provide information about our services. Enters referral information into database accurately and completely, documenting all contact and conversations regarding referral.
Immediately evaluates referrals and directs them to proper individual for assessment. Processes referrals not requiring outreach or actual assessment and follows up with disposition quickly. With Admissions Team, identifies strategies for follow-up on pending referrals and prioritizes same.
Verifies financial information with payor, contacting Accounting office for questionable cases.
Achieves response rates as established with Admissions Director. Actively identifies potential barriers to admission and ways to resolve.
Facilitates admission decision, consulting with Admissions Outreach Nurse, Director, physician, or any other clinicians as necessary. Tours family on recommended nursing unit prior to admission. Communicates with Admissions Director when beds are unavailable.
Communicates necessary information to departments about all anticipated admissions.
Achieves average monthly census goals established with Admissions Director. Completes monthly reports of referrals/admissions/conversion rates. Advises Director of referral concerns and makes appropriate suggestions to increase referrals.
