Admissions / Outreach Coordinator

Daylight Healthcare Services is currently seeking an enthusiastic and dedicated Admissions / Outreach Coordinator who will contact Service Coordinators, families and obtains required information and admit supported people (people with developmental disabilities) into the agency.

The work schedule: Work one day in the office and other days remotely.

JOB DUTIES & RESPONSIBILITIES:

• Coordinates the admissions inquiry process: Contact Service Coordinators by phone and in-person, meets with families, and communicates relevant information to facilitate admission process and follow up with families.
• Identifies strategies for follow-up on pending referrals and prioritizes same.
• Obtains all information necessary to make admission decision including people data, insurance information and medical diagnosis.
• Following the admission guidelines, approves admissions and communicates to the referral source.
• Assist in obtaining required admission information and signatures. Collect, assemble and review admission documentation for accuracy and completion.
• Finalize the welcome and move-in process, which includes but is not limited to, following up with family to determine satisfaction with service.
• Respond to requests for medical record information by insurance companies, Medicare, Medicaid, or healthcare providers as authorized and in accordance with Health Insurance Portability and Accountability Act (HIPAA) privacy and security requirements
• Perform other duties as assigned.

EDUCATION & EXPERIENCE REQUIREMENTS:

Education

• Associates degree. Can be substituted by experience.

Experience:

• Two years of experience working in-take administration.
• Knowledge of current Medicare/Medicaid regulations regarding admission/discharge requirements for DDA.
• Must be proficient in email and spreadsheet applications.
• Must possess the high knowledge of seeking new referral to the agency.

Job Types: Part-time, Commission

Pay: $18.00 - $25.50 per hour

Expected hours: 20 – 40 per week
