SUMMARY
The Scheduling Coordinator is responsible for ensuring that patient care is coordinated and managed appropriately. . Respects and promotes the mission, vision, and values of VNA Health.
ESSENTIAL JOB DUTIES AND RESPONSIBILITIES
- Receives case referrals. Reviews available patient information, including disciplines required, to determine home care needs. Makes Assignment According to patient needs.
- Ensures that authorizations are received and requested as needed for new referrals.
- Assigns patients appropriately for continuity of care; ensuring productivity standards are met.
- Responsible for coordination of all patient visits.
- Coordinates and manages the monthly schedule of nurses and home health aides.
- Support intake coordinator; receiving and processing new referrals, triaging for skilled need and same day visits.
- Initiates conferences with nurse manager or program director regarding any questions about individual eligibility for services.
- Receives and triages phone calls from patients and refers all sources, as related to scheduling.
- Complies with accepted professional standards and principles.
- Has working knowledge of state, federal, local and accreditation regulations for the delivery of home health services.
- Able to prioritize and facilitate interventions that effectively solve or prevent problems related to day to day operations

POSITION QUALIFICATIONS
- College degree preferred or equivalent experience.
- General front and/or back office experience required, with previous support of management highly preferred.
- Experience in handling phones, medical records, schedule changes, medical billing and data entry in a medical environment preferred.
- Demonstrated ability to perform multiple tasks in a fast-paced busy environment. Ability to deal with rapidly changing priorities.
- Must have high level of interpersonal skills to handle sensitive and confidential situations.
- Must have strong computer software aptitude including Google Docs and Excel
- Electronic medical records experience, not required
- Ability to communicate compassionately and effectively about end of life issues.
- Demonstrated ability to prioritize workload.
- Knowledge of state, federal, local, and accreditation regulations pertinent to job responsibilities.
- Excellent written/verbal and interpersonal communication skills.
- Spanish Speaking highly preferred (bilingual Spanish)
