Job description

  • Review and investigate Incident/Accident report forms and review they are legible. i.e. collect data from the charts, gather statements from staff, summarize the investigation in a summary note.
  • Review and investigate I/A reportables. i.e. Collect data from the charts, gather statements from staff, complete a full investigation, write reports for the appropriate ICF/IID reporting department MDCPS, MSDH, MFCU and DRM.
  • Evaluate and review team treatment plans and team meeting notes for correct grammar and spelling. Determine that documents reflect active treatment and compliance with state regulations.
  • Evaluate all incidents for reportable occurrences.
  • Drafts external incident reports for review and submission by the Director of Medical Records/Performance Improvement.
  • Analyzes all incident reports and assigns appropriate category codes as outlined by corporate guidelines.
  • Maintains a log of all Incident Reports
  • Identifies trends and makes recommendations to the Performance Improvement department.
  • Ensures immediate reporting of injuries and/or unsafe conditions to supervisor and completes any related documentation.
  • Participate in treatment team process to evaluate programs and make recommendations to enhance quality of care.
  • Ensure all treatment plans for the PRTF coincide with policies, standards and expectations.
  • Review/revise/edit treatment plan database, as necessary, to maintain regulatory compliance.
  • Audit charts for utilization review with needs identified and notify the appropriate staff members.
  • Assist in the utilization review of PRTF patient charts with recommendations to the treatment team.
