Porter-Starke Services, Inc. is a Community Mental Health Center (CMHC) based in Valparaiso and serves northwest Indiana with additional offices in Portage, Knox, and LaPorte. Porter-Starke also operates Marram Health Center, a Federally Qualified Health Center (FQHC) with two offices in Gary and Hebron.

JOB DUTIES:
- Completes initial assessments for new Wraparound referrals
- Completes a comprehensive assessment with enrolled youth and families in accordance with the Wraparound model
- Facilitates Child and Family Team Meetings
- Monitors progress towards goals and all services authorized for youth’s Wraparound care
- Completes, regularly updates and oversees implementation of a Plan of Care with each Wraparound Team outlining needs, goals, and strategies for participants
- Gathers, records, and reports all necessary data per DMHA requirements
- Provides Case Management Services when necessary, not to exceed State-Mandated caseload sizes for Wraparound Facilitators
- Participates in regular Process-Based Supervision to ensure adherence to the Wraparound model
- Assures services are provided in a manner consistent with strength-based, family driven, and culturally competent values
- Provides consultation and education to partners and participants regarding the values and principles of the Wraparound model
- Attends staffing, clinical supervision, in-services, and staff meetings
- Typing and basic computer usage
- Performs other duties as assigned.
COMPETENCIES:
The staff member must demonstrate knowledge of:
- Individual, group, family, and team dynamics.
- Developmental theory, clinical symptoms, and interventions relevant to the Wraparound model
- Accessing and coordinating appropriate referrals, services, and entitlements for youths
- Current policies and procedures of the agency, relevant DMHA standards, and professional standards.
- Trauma-Informed Care.
- Screening and Assessment for risk of harm to self and others.
- Signs and symptoms of psychiatric disorders.
The staff member must demonstrate skills in:
- Assessing child and adolescent behavior, providing crisis intervention as needed
- Establishing rapport with students, their families, and other Wraparound team members
- Individual and group interventions
- Providing relevant information to team members to assist them in managing challenging situations and fulfill their responsibilities under the Plan of Care
- Time management and productive work habits
- Documenting interventions
- Typing and basic computer usage
- Advocating for client's needs.
QUALIFICATIONS:
- Bachelor’s degree in psychology, social work, or related human services field is required
- Valid Driver’s License and automobile insurance
- Related experience equal to or greater than 2 years
- Experience in crisis intervention and working with children is preferred
