Adult & Child Health is an integrated healthcare provider in Central Indiana. About this role:
As a Wraparound Facilitator, you will facilitate the Wraparound process, which is a strengths-based and team-based approach in order to support high-need youth and families. You will partner with families on building their support systems, as well as working to get an understanding of the needs below the surface that are leading to their struggles, focusing on the whole family. Facilitators lead the process, which includes leading monthly Wraparound Team Meetings and communicating with team members weekly, as well as maintaining the plan to ensure fidelity to the Wraparound model.
The Location and hours: A&C is based in Central Indiana. For this position, we service the following county: Marion
Hours are M-F 8-4pm or 9-5pm with some evenings and flexible scheduling available.
Responsibilities include but not limited to:
- Leading a team including family members, providers, and natural supports
- Maintain communication with team members to ensure follow-through of tasks and collaboration
- Leading crisis planning with families and supports, as well as ongoing updates to ensure safety
- Maintaining documentation in Adult & Child and DMHA systems
- Engage and/or support consumers and their families to provide encouragement on beginning and/or continuation of services
- Identify engagement barriers and possible solutions as well as utilizing consumer strengths to encourage and inspire them to reach their full potential.
Requirements
- Bachelor's degree in Social Work, Psychology, Human Services, or other related field
- Two years experience with children ages 6-17 that have been identified as Seriously Emotionally Disturbed (SED).
- Must hold a valid driver's license with acceptable driving record and provide own, insured, transportation
- Experience with Systems of Care preferred
- Available to work weekdays and evenings; No "on-call" after hours
- Ability to maintain professional relationships with a variety of individuals
- Comfortable working in a range of settings, including: homes, the community, and schools
- Strong organizational skills
