Qualifications
Position Summary
The Activity Coordinator position develops, coordinates and implements a program of activities that take place in the Wellness/Activity center, intended to provide mental, physical and social stimulus to patients, based upon identified needs, interests, capabilities, and well-being of each patient. The coordinator will also act as a liaison between the medical center staff and patients, community guests and professional visitors that may provide services for Wellness programs following state regulations and established policy and procedures.

  1. Ability to develop, implement creative therapeutic activities to help with different physical, mental, or psychosocial needs, such as socialization.
  2. Develops a group activity plan, which meets the interests, needs and capability of participants
  3. Good communication skills among patients, providers, staff and administration.
  4. Must be able to follow company policies and procedures.
  5. Be able to work flexible schedule and location.
  6. Ability to effectively relate with medical staff, peers, managers, supervisors, patients and employees.
  7. Supervise groups, leads games, exercise programs and other routine activities as assigned.
  8. Schedules, distribute and post the weekly/monthly activities for patients to be aware of future events.
  9. Host social events including: Birthday Parties, Bingo, Domino, Social Dances, among others.
  10. Responsible for decorating event room or area according to event theme.
  11. Coordinates transportation for the patients attending activities.
  12. Ensure equipment is maintained in good condition and the center is kept clean and organized.
  13. Reports any issues/recommendations made by patients to Administrator.

Complete and submit monthly reporting of center activities:
  1. Complete monthly activity reports, including detailed information about center activities (Reports must be submitted to Center Manager by the 5th of each month).

Monthly Reporting Inform will include:
  1. Photos of activities
  2. Participation sign-in sheets
  3. Flyers
  4. Projected calendar of events for upcoming month.
Supervisory Responsibilities
This position has no supervisory responsibilities.
Physical Demands
The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.
While performing the duties of this job the employee is regularly required to work standing up, walk, use hands to operate tools and equipment and must be able to exert regularly up to 10 pounds of force, frequently exert 30 pounds of force and occasionally exert 50 pounds of force to constantly perform the essential job functions. The employee will be frequently required to reach with hands and arms, bend, balance, kneel, crouch, crawl, push, and pull. Specific vision abilities required by this job include close vision, distance vision, peripheral vision, depth perception and ability to adjust focus. The employee is subject to environmental conditions; protection from weather conditions but not necessarily from temperature changes. The worker is subject to noise; there may be sufficient noise to cause the worker to shout in order to be heard above ambient noise level.
Required Education and Experience
  1. Excellent customer service, interpersonal, organizational, and multi-tasking skills.
  2. Leadership and public speaking skills.
  3. Silver Sneakers certification or obtain within 6 months of employment
  4. Knowledge as a Wellness/Activities Coordinator, Educator, Instructor.
  5. Sensitivity to the needs of an aging population.
  6. Working with seniors: 1 year (Preferred)
  7. Formal activity program: 1 year (Required)
  8. Physically able to stand for extended periods of time
  9. Bi-lingual fluency in English and Spanish
  10. High School Diploma
  11. CPR (BLS)
  12. First Aid
  13. Good writing, verbal, and communication skills
  14. Able to respect and always maintain patient confidentiality.
  15. People-oriented - enjoys interacting with people and working on group activities
  16. Ability to motivate patients
  17. Develop and deliver an exercise routine tailored to patient needs and capabilities
  18. Professional appearance
  19. Advance computer knowledge
Preferred Education and Experience
  1. Relevant or any other job-related vocational coursework.
