Position Overview:
We are seeking a Case Manager to help new clients enroll in public benefits for which they are eligible, assist with acculturation to the US, help clients learn and use the public transportation system, identify unmet client needs, work as a member of a multidisciplinary team, and be highly proactive.

Responsibilities:

• Conduct outreach activities in communities served.
• Help new clients enroll in public benefits for which they are eligible, including SNAP, Medicaid, and TANF
• Assist with acculturation to the US, providing information and resources on topics such as US laws and customs, healthcare, education, and employment
• Help clients learn and use the public transportation system, including providing information on routes, fares, and schedules, and accompanying clients on trips as needed
• Identify unmet client needs, including health, housing, education, and employment, and work with clients to develop and implement a plan to address those needs
• Work as a member of a multidisciplinary team, including Case Managers, employment specialists, literacy intervention specialists, and mental health professionals, to ensure that clients receive coordinated and comprehensive services that meet their needs
• Be highly proactive, reaching out to clients regularly and monitoring their progress towards goals, and taking a proactive approach to identifying and addressing emerging client needs
• Maintain accurate and up-to-date client records in case management software and databases
• Participate in program evaluation and continuous improvement efforts
• Attend relevant community events and trainings to stay current on best practices and emerging trends in the field
• Other duties as assigned by the Program Manager

Qualifications:

• Bachelor's degree in social work, psychology, or a related field preferred
• At least two years of experience providing case management services to immigrants and refugees, preferably in a non-profit or community-based setting
• Knowledge of public benefits programs, including SNAP, Medicaid, and TANF
• Knowledge of acculturation and integration issues facing immigrants and refugees in the US
• Persons with lived experience highly preferred
• Familiarity with the public transportation system in the Chicago area
• Excellent communication and interpersonal skills, with the ability to establish rapport with clients and build relationships with external partners
• Strong organizational skills and attention to detail, with the ability to manage multiple tasks and projects simultaneously
• Ability to work independently and as part of a team
• Familiarity with case management software and databases
• Fluency in English and Spanish or Arabic required.
