Housing Works fights for funding and legislation to ensure that all people living with HIV/AIDS have access to quality housing, healthcare, HIV prevention, and treatment, among other lifesaving services.
Benefits:
We have three comprehensive healthcare plans to choose from based on your priorities and budget. Housing Works covers most of the plan; you pay a portion, based on your salary. Staff begins accruing PTO immediately for a total of up to 30 days earned in the first year. We offer employees an educational benefit. This position is included in a bargaining unit of Housing Works' New York City employees represented by a labor union known as the Retail Wholesale and Department Store Union ("RWDSU"). Accordingly, the RWDSU has the exclusive right to bargain over the terms and conditions of employment related to this position and this position may become covered by the terms of a collective bargaining agreement (a "CBA") between Housing Works and the RWDSU

 

Compensation Range: $21/hr - $23/hr commensurate with experience 
Overview:  

Using a one to one, person-centered counseling model, Community Oriented Recovery and Empowerment (CORE) program seeks to improve the functioning of individuals living with mental illness, histories of chemical and alcohol dependence, or physical disabilities, helping them achieve personal goals in the areas of recovery, employment or independent functioning. Under the supervision of the Clinical Director, the Rehabilitation Counselor will provide HARP services with participants referred from Care Management Agencies/in-house HARP eligible clients in following areas: Psychosocial Rehabilitation (PSR); PSR with Educational focused, and/or PSR with Employment focused. These services are intended to contribute to the reduction of unnecessary hospitalization/ institutional level of care and improved societal integration of those served. Service delivery is designed to be flexible and individualized and is focused on the promotion of skill development and identification of community supports in order to overcome deficits related to function and motivation.


Responsibilities:

Engage individuals in shared decision making and recovery planning

Demonstrate empathy towards population.

Provide services as outlined in each participant's Plan of Care

Support clients in identifying and creating goals and developing personal goals with the skills, strengths, supports and resources to aid them in achieving those goals

Collaborating with clients' case manager or treatment team in determining the steps needed in order to achieve these goals and self-directed recovery

  Address behavioral health barriers that would prevent client from pursuing employment and educational goals

Assist client with skill development related to academic survival skills, stress management, self-advocacy in the educational setting.

Assist client in developing skills related to the job application and interview process and "soft skills" related to employment.

Provide support in navigating application, enrollment, registration, and financial aid processes

Connect client to job training and educational programs available in the community, disability rights and resources, tutoring, mentoring services, and social supports

  Facilitate Psychosocial Rehabilitation Groups, if needed

Support clients' vocational/educational choices and assist them in choosing a job that matches their strengths, overcoming job-related anxiety by reviewing job applications, and providing interview tips.

Keep progress notes and develop Individualized service plan based on client's vocational/educational interests

Hold case conferences as needed

Perform other duties as assigned by supervisor

Minimum Requirements 

Bachelor's degree required; with relevant experiences in supporting individuals pursuing educational/vocational goals

Majors in Education, Psychology, Sociology, Social Work, Human Services, counseling, or other related fields preferred

Experiences with serving people with mental illness, alcoholism, or substance use preferred
