The Team Coordinator is responsible for assisting the clinical leadership team in day-to-day clinical operations for the branch to ensure a seamless and organized hospice care system. The Team Coordinator maintains a comprehensive working knowledge to serve as a resource for field staff.
Essential Job Functions/Responsibilities
1. Coordinates training for new hires; to include EMR training, orientation books, and clinical supplies.
2. Works with the clinical leadership team and staff clinicians to gather, collate, and report patient care summary information to Case Managers.
3. Regularly participates in interdisciplinary group meetings and assures through attendance and record reviews that meetings are conducted in accordance with Agency and CMS regulations.
4. Maintains comprehensive working knowledge of patients and ensures the needed documents are received and organized in the system.
5. Works with the clinical and operational leadership to ensure proper staff utilization and confirms that required tasks are plotted accurately on the schedule for the field staff and physicians. Also checks to ensure visit documentation is being completed in a timely manner.
6. Assists with Quality Assurance of referral documentation and with all aspects of the intake process as needed.
7. Track supply orders for the branch to ensure availability of sufficient inventory for clinicians.
8. Communicates schedule with triage and on-call staff.
9. Responsible for responding to all patient and community requests and any field requests in a timely manner.
10. Maintains comprehensive working knowledge of community resources and assists customers in accessing community resources should services not be provided by Heartford Hospice.
11. Participation in Quality Assurance/Performance Improvement Program and Hospice sponsored training.
Position Qualifications
1. At least two years of experience in health care managed care coordination, preferably in hospice care.
2. Complies with acceptable professional standards and practice.
3. Demonstrates good communication, negotiation, and public relations skills.
Demonstrates autonomy, organization, assertiveness, flexibility, and cooperation in performing job responsibilities.
