Support Coordinator
Duties:
- Conduct assessments and develop individualized service plans for clients
- Coordinate and monitor the delivery of services to ensure client needs are met
- Provide support and guidance to clients in accessing community resources and services
- Assist clients in budgeting and financial management
- Collaborate with other professionals, such as therapists and healthcare providers, to ensure comprehensive care for clients
- Maintain accurate and up-to-date client records
Skills:
- Strong organizational and time management skills
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Knowledge of budgeting principles and practices
- Familiarity with behavioral health services and resources
