Position Description:
The Senior Community Outreach Worker II reports to the Program Director and is responsible for conducting outreach and engagement activities targeting a specific population. This role involves referring individuals to primary care medical services at the Center for Community Health (CCH) in Downtown Los Angeles and coordinating with the street medicine team. The primary focus is on developing relationships with at-risk or homeless individuals and facilitating linkages to various services, including those related to chronic health conditions, substance use, behavioral health, and other social needs. The Senior Community Outreach Worker II will be accountable for achieving daily and weekly outreach goals, resulting in completed Clinic encounters.
Principal Responsibilities:
- Develops and implements effective outreach strategies to engage and build relationships with homeless individuals and those at risk of homelessness.
- Conduct street outreach activities, actively engage with individuals, and provide information about CCH and street medicine team programs and services to encourage participation.
- Ensure a seamless transition of individuals from street and mobile unit encounters to the Center for Community Health and the street medicine team through a warm handoff process.
- Maintain accurate records of all outreach activities, including encounters, interactions, and outcomes.
- Assist with clerical duties such as intake and assessment, scheduling appointments, and tracking outcomes of clients.
- Actively participate in team huddles and meetings to share insights, updates, and coordinate outreach efforts.
- Assist individuals in accessing a range of services to address their diverse needs and requirements.
- Perform any other duties as assigned by the Program Director.
Requirements:
- A minimum of two years of work experience in conducting outreach activities, preferably in a similar context.
- Ability to travel between different outreach sites as required to reach the target population.
- Proficiency in working collaboratively within a team structure and contributing to a collective goal.
- Strong verbal communication skills to effectively engage with clients and team members.
- Consistency and punctuality in performing job duties, including timely arrival at outreach sites and clinics.
- Experience in working with clients and staff from diverse socio-economic, ethnic, and cultural backgrounds.
- Ability to establish rapport with clients that conveys genuine concern for their well-being and needs.
- Capability to follow complex instructions and adapt to changing situations in the field.
- Bilingual (English/Spanish)
