Job Description
Position Summary: MSW will work as part of an interdisciplinary team completing Psychosocial assessments for patients in home setting. MSW will develop an individualized plan of care to assist patient and caregivers with linkage to community resources, short term counseling support and long term planning in collaboration with patient’s Primary Care Physician. Experience with visiting patients in home setting recommended but not required. Master’s level licensure LCSW or LICSW is required.
