Social Worker: Case Manager

Guardian Healthcare is a certified nursing agency. Our Family Caregivers division, which focuses on Adult Family Care (AFC) has an exciting opportunity for a Case Manager in the Boston area.

AFC is a program which facilitates patients receiving care from their loved ones in the comfort of their homes. Our agency organizes and manages care, including ensuring that caregivers are qualified and approved.

Case Manager Position Overview:

This position is responsible for the evaluation of clients and caregivers. Also, for the initial and ongoing assessment of both setting and care provider.

It offers a nice balance between case management, desk work and on-site visits in Shrewsbury and neighboring towns.

Hybrid Position

Desk work may be done from home. There will be a need to access the office for meetings, printing, uploading documents etc.

Bilingual

The successful candidate must be conversant in English and Spanish.

Core Tasks:

• On-site visits with each member at the qualified setting to ensure the patient is well cared for and legal requirements are met.
• Monitor client activity, condition and provide case management services.
• Evaluate, support and train the AFC caregiver.
• Ensure implementation of the AFC plan of care.
• Scheduling appointments for referrals.
• Checking insurance eligibility.
• Submission of necessary information and communication with the patient’s doctors.
• Assist client with other Social Work Services.
• Review and file necessary documentation.
• Facilitate staff meetings to share information and problem solve.
• Community outreach including presentations.

Job Type: Full-time

Pay: $21.00 - $25.00 per hour
