QUALIFICATION STANDARDS:
Knowledge, Skills and Abilities: Requires demonstrated professional knowledge and skills necessary to plan, organize, and develop support services for our current residents and their families; the ability to establish and maintain effective professional working relationships with residents, families, and staff; knowledge of the inquiry and admissions process; fundamental knowledge and skill in marketing; effective verbal and written English communication; lift and carry up to 30 pounds.
Education: Graduate of an accredited institution with a bachelor’s degree in social work or extensive equivalent experience in the field. 3.
Experience: Prior social work experience in the geriatric field preferred.
Certificate/Licenses: Bachelor’s Degree in Social Work and valid driver’s license.
JOB DESCRIPTION:
• Conduct continuous family and resident counseling including crisis intervention, facility adjustment, resident status changes, etc.
• Assess each resident within seven (7) days of admission and on a quarterly basis.
• Coordinate and lead education for family support groups.
• Attend monthly resident council meetings (when invited by council) and address resident concerns.
• Coordinate room changes; notify residents & families.
• Coordinate changes such as room transfers, cable and telephone , pharmacy, laundry, etc.
• Develop and coordinate staff training sessions such as Residents’ Rights, Guest Relations, and Resident Dignity.
• Assist discharged residents and families with placement options.
• Coordinate Discharge Planning Service (i.e., discharge time, special needs) and initiate a discharge follow-up call within forty-eight (48) hours after discharge.
• Initiate MA 103 & Opening letter; responsible for completing forms & sending to Dept of Welfare. Check PASSAR on admission & complete 408’s on targeted residents.
• Direct and assist admissions coordinator with inquiries and facility tours, marketing, and networking.
• Assist with admission sign-in and discharge process as needed.
• Establish and implement a follow-up visit system for all new residents per individual care plans.
• Establish and coordinate resident welcoming committee.
• Visit all new residents and families within 24 hours of admission to facility.
• Participate in quarterly care plan conferences, including invitations to families and follow-up review with residents. Assure good representation at care conferences.
• Complete Medical Assistance application with family members as. Complete MA 51’s.
• Chair and participate in weekly rehab meetings. (Facility Name Goes Here)
• Participate in monthly Behavior Management meetings and follow up with any concerns.
• Coordinate completion of Medical Assistance applications.
• Practice safe working habits.
• Update department’s policies and procedures manual at least annually.
• Participate in Marketing activities.
• Identify Target residents/408; complete reporting forms within 48 hours of admission.
• Write necessary plans of care.
• Address resident and family concerns and/or complaints.
• Complete assigned MDS sections.
• Complete PA-PASRR-ID forms within 48 hours of admission.
• Complete Notice of Non-Coverage letters and issue to residents/families.
