Overview: Salaried LSW with UMWA. Monday-Friday Schedule!

Compensation: Salaried

Job Responsibilities:

• Perform home/facility visits to Funds’ beneficiaries to assess a beneficiary’s home environment, medical and social needs, and support system. Home visits are to be completed between 9AM and 5PM, Monday – Friday, excluding holidays.
• Complete general health assessments.
• Assist with coordination of services and benefits.
• Provide education on options and assist with long term planning.
• Assist caregivers to better understand the beneficiary’s needs, benefits, and available resources. Caregiver assistance will be provided both informally and through a Funds’ provided Caregiver Support Program. A caseload will be built up and consistently maintained for the Caregiver Support Program.
• Identify beneficiaries who have medical or social needs and make referrals to the Funds’ care coordination and health support programs or to appropriate community agencies.
• Complete a minimum of 5 beneficiary home/facility visits per week (i.e., approximately 240 visits per year). This number may be adjusted by the Funds due to special projects or other reasons. Home/facility visits are scheduled based on outreach, referrals from the Medical Management vendor or community agencies, through involvement in formal Field Health initiated programs, and/or self-referral. All referrals should be responded to within 1-2 business days of referral receipt. Additional beneficiaries may be aided over the phone when a home/facility visit is not accepted by the beneficiary. Home/facility visits are required to be documented as communicated by Funds’ personnel.
• Assist beneficiaries who are applying for assistance from local, state, and federal programs

Requirements:

• Education: Bachelor of Science in Social Work (BSW) or comparable undergraduate degree required; Master of Social Work (MSW) preferred.   
• Licensure Requirement: LSW in both Ohio and West Virginia
• Minimum of 5 years of relevant experience and must have knowledge in health issues related to geriatrics and experience in health education.
