General Summary/Responsibilities:
SOCIAL SERVICES
SHIFT: Full-Time
HOURS PER PAY: 80 hours pay period
The medical social worker provides direct social work intervention, psychosocial assessments, transition planning and counseling to patients and their families. The medical social worker assesses the patient’s social, emotional, financial, cultural and family situation and interprets this information to the physician, nursing staff, and other care team members to facilitate appropriate transition planning. The medical social worker assists patients and families in making appropriate linkages to community resources and provided consultation to those seeking input and guidance in efforts to resolve patient concerns. Transition planning and social work services are provided to inpatient and outpatient populations of the hospital.
Qualification
Education: Master’s Degree in Social Work required.
Licensure: Licensure with the State of Ohio Counselor, Social Work and
Marriage and Family Therapist Board. LSW required, LISW preferred.
Certification: None required
Experience: Previous experience in a health care setting or other social service agency required. 3-5 years of experience preferred.
Skills/Other: Must possess strong clinical, interpersonal skills. Must possess strong communication skills, both in verbal skills and in writing skills. Must possess ability to work in a busy, stressful environment, and be able to work independently. Current knowledge of medical terminology, Medicare/Medicaid and third party payor requirements preferred. Basic Computer Skills required.
