The Licensed Social Worker is responsible for directing and monitoring social work services in the center and under the guidance of the Social Services Director.
Some of the job responsibilities of the Licensed Social Worker include:
- Promoting the psychosocial wellness of the residents.
- Actively participating in resident assessment and discharge planning to allow the resident to optimize healthcare benefits and promote safe discharges back into the community.
- Maintains and communicates the center's discharge calendar, working closely with the Interdisciplinary Team.
- Coordinates and arranges for resident's care planning meetings ensuring that resident and responsible persons are involved as able.
- Initiates discharge planning immediately upon admission by identifying resident goals and barriers to community re-integration.
- Develops plans in cooperation with the resident accordingly to address barriers. Works closely with community resources such as Home Health, Hospice, local housing authorities, shelters, and day treatment providers to ensure resources are available as needed per discharge plan.
- Responsible to make sure that a Notice of Transfer and Bed Hold policy are provided to residents upon transfer out of the center. Provides required Ombudsman notification of transfers out of the center per regulation.
Minimum Qualifications of the Licensed Social Worker:
- Minimum of Bachelor's Degree in Social Work, Master's degree preferred.
- Current Social Work License in the state of MA.
- Three years’ experience in geriatric, hospital, administrative, or community social work. Ability to read, write and speak English daily. Other languages helpful but not required.
