Position Summary/Essential Duties:
Collaborates with all members of the health care team, patient and family (or significant others) to coordinate timely and efficient assessment of psychosocial and discharge planning needs and formulation of patient and family centered discharge plans. Serves as a resource within the social work department and the hospital.
1. Provides psychosocial assessment / intervention to assist patients/families (including clinically complex).
o Provides age specific care to patients/families in all age groups.
2. Develops, documents and implements a comprehensive plan based on identified psychosocial needs and/ or discharge needs.
o Develops plans with attention to individual patient and family goals
o Provides linkage to community resources and UH network services to facilitate optimal care; and serves as liaison to community agencies.
3. Communicates/collaborates with the work team using a multidisciplinary approach.
4. Provides guidance and serves as consultant to staff and other disciplines regarding complex psychosocial and/or discharge planning issues/barriers.
5. Assists with recruitment, and orientation/mentoring/education of new staff and or social work interns.

Education Requirements:
Master's degree from an accredited school of Social Work
License Requirements:
Licensed by the State of Ohio Counselor, Social Worker, Marriage and Family Therapist Board at the LISW or LISW-S level; Attainment of LISW-S level encouraged.
Position Details/Preferred Qualifications:
Must have at least 1-3 years of experience. Previous work with the perinatal population is strongly preferred. This Social Worker will be working with mood and anxiety disorders in pregnant women, as well as postpartum mood and anxiety disorders. Main responsibilities include performing 5-6 hours of IOP group therapy per week, new patient intake assessments, completing all patient documentation, care coordination, participating in weekly treatment team meetings, marketing the program, and networking within the perinatal care community.
