Social Service Coordinator
Classification: Exempt/Salary
Salary range: $40,000 - $45,000/ Dependent on Experience; SIGN ON BONUS: $1,000
License preferred: Yes
Availability: Monday to Friday, 8:00 am to 5:00 pm, with weekends/nights availability as needed.
Job Summary:
The primary purpose of this position is to assist in planning, organizing, implementing, evaluating, and directing the overall operation of the Social Service Department in accordance with current federal, state, and local standards, guidelines and regulations, and our established policies and procedures. The Social Service Coordinator also educates residents and families on available services, develops contact with services providers and agencies for resident referrals and monitor provision of services.
Essential Duties and Responsibilities include, but are not limited to:
- Assess social service needs of residents, determine goals, and provide services to meet identified needs.
- Minimum of forty hours per week, or as many hours as required to meet job expectations, based on a Monday through Friday schedule, including weekend, holiday and other alternate hours as required.
- Visits the facility on second and third shifts as required.
- Counsel residents and family members as required to cope with changing physical/medical conditions, care level changes, death, etc.
- Attend care planning meetings and staffing to evaluate resident's problems, needs, and to plan solutions.
- Advise on individual activities based on social, emotional, and spiritual needs of residents, and meeting special needs created by various conditions or impairment.
- Orient new Wesley Willows residents and their families with the accompanying adjustments.
- Develop social history of each Wesley Willows resident and document in charts social service needs and progress notes.
- Co-Chairperson of the Resident Review Committee and serve as resident advocate.
Administrative
- Develop and implement both short- and long-term objectives.
- Plan, develop, organize, implement, evaluate, and direct the department's programs and activities.
- Establish resident relations program; handle all resident concerns promptly.
- Ensure compliance with all federal, state, and local laws and regulations.
- Maintain a safe and productive work environment.
- Make routine inspections of the departments to assure that established policies and procedures are being implemented and followed.
- Participate in facility surveys (inspections) made by authorized government agencies. Assist in the development of a plan of correction for deficiencies noted during survey inspections.
Physical Resource Management/Safety
- Ensure that adequate supplies and equipment are on hand to meet the day-to-day operational needs of the facility and residents.
- Implement and evaluate a safety plan, in conjunction with the facility administrator, which will ensure the health, welfare, and safety of residents, staff and visitors.
- Develop, implement, and evaluate a departmental fire and disaster plan to protect the safety and welfare of residents, staff and visitors.
- Review accident/incident reports and establish an effective accident prevention program in conjunction with the facility safety committee.
- Ensure that all personnel follow established regulations governing the use of labels and MSDSs for hazardous chemicals.
- Ensure that all personnel performing tasks that involve potential exposure to blood, body fluids, or hazardous chemicals participate in an in-service training program prior to performing such tasks.
Minimum Education/Skills/Experience/Credentials:
- Bachelor’s degree in social work, the social sciences, or a related field preferred.
- Prefer 2-5 years’ experience in activities or program coordination, counseling, and/or social work.
- Good interpersonal skills both written and verbal.
- Ability to interface with residents, family members, and employees in a professional manner.
- Computer literacy required.
- Professional License in Social Work
Other regulated requirements:
Must be willing to undergo pre-employment background check, including (MVR) moving vehicle records.
Must have or be willing to receive the Flu-Shot Vaccine, and TB testing.
Must be willing to undergo pre-employment drug screening.
Must be willing to undergo biometric screening.
