Responsibilities

The Self-Directed Services Coordinator will act as the primary Liaison between the individuals, families, DDRO, OPWDD, and Fiscal Intermediary

agencies. The SDC is responsible for the supervision and training of the agency Brokers and will provide self-directed training as needed. Assures

that each Broker provides the maximum units of services to each assigned individual. Responsible for ensuring that all areas of responsibility comply.

with OPWDD, OMIG, and OIG is submitted by the department's agency deadlines. Completes routine reports, special projects, serves on committees.

Qualification

Requires a Bachelor's degree and one year experience working with people with developmental disabilities and two years of supervisory experience.

Must have a valid New York State Driver's License, a reliable vehicle, meet Lifetime Assistance vehicle operator requirements and agency

standards to transport service recipients.

Salary

$22.87- $26.50
