Service Coordinator

Best Health Care Services, is seeking a Service Coordinatorwho is experienced and able to handle incoming referrals and communicate in a professional manner with clinicians, Nurses and all Staff , Insurance coordinators, county eligibility workers, family members, Patients. Be able to have computer skills and proficient in English. Self organized and work efficiently for time sensitive documents and reports to keep us compliance within agency and state guideline.

Responsibilities:

• Handle incoming referrals and prepare packages for admissions and continuous supervisory visits
• Assisting Medicaid coordinator with all time sensitive documents for our insurance coordinators for client service approvals .
• Manage and Coordinate with office and field staff the daily activity assigned.
• Work closely with Administrator ,Clinical Case Manager, all Staff
• Provide information about the products and services to customers
• Schedule / Troubleshoot and coordinate service visits for our field Nurses.
• Document and update customer records based on interactions
• Develop and maintain plan of care documents based on Services provided for Waiver patients.
• Prepare and monitor all documents for field clinicians to visit patients
• Assist in follow up with coordinators and direct insurance coordinators

Qualifications:

• Previous experience in home health and customer service, sales, or other related fields is a plus
• Ability to build rapport with clients
• Ability to prioritize and multitask
• Positive and professional demeanor
• Excellent written and verbal communication skills
• Must be Organized and self motivated
• Job Type: Full-time
• Kinnser / Wellsky knowledge is a plus.

Job Type: Full-time

Pay: $18.00 - $22.00 per hour

Expected hours: 40 per week
