Description
Reports to: Care Coordination Assistant Manager
Job Objectives:
Provide outreach for the Care Coordination Department and care management services to individuals enrolled in the Health Home Program, while also mentoring other support staff within the department.
Perform duties while actively promoting an environment consistent with MHA's Cultural Equity Policy and WELCOME Orange
Responsibilities:
• Attend bi-weekly Care Coordination team meetings.
• Attend trainings and meetings relevant to Health Homes.
• Responsible for outreach process of all Health Home referrals including but not limited to: sending information letters, phone contacts, street contacts, researching referral locations and maintaining referral lists by outreach and hiatus status.
• Assist new program participants with health home services until they are assigned a permanent care manager, as needed.
• Enter all referrals into the electronic health record, including FCM and AWARDS.
• Complete New York State HARP assessments and reassessments, as needed.
• Maintain all client files according to program policies and procedures, including assisting with internal audits, as needed.
• Establish and maintain linkage with appropriate community agencies including but not limited to: service providers, managed care organizations, and the health home.
• Provide community agencies with presentations, in person or via zoom, on the services that Care coordination can provide.
• Communicate with liaisons from Garnet Health for Behavioral health and Emergency Department referrals and complete all necessary documentation for referral process.
• Conduct MAPP, PSYKES, CIN/EPACES reports, as needed.
• Represent MHA at meetings as necessary.
• Provide oversight to other intake or case aide support staff, as needed.
• Assist intake staff or case aide support staff when they are completing a difficult intake (i.e if a new program participant is experiencing a crisis)
• Answering general care management, health home, or intake/outreach questions for support staff and ensuring a manager or supervisor is informed if the question or situation is more complex.
• Assisting with training and/or shadowing of new intake workers and/or case aides.
• Reviewing time management strategies and deadlines with support staff, as needed.
Other Duties:
• In the spirit of teamwork, answer MHA's main office phone as needed.
• Participate in MHA's committees as needed.

Requirements
Qualifications:
• Bachelor's Degree in related field with 1 year of experience providing direct services to medically underserved populations OR
• Associate's Degree in related field with 2 years of experience providing direct services to medically underserved populations OR
• High School Diploma with 4 years of experience providing direct services to medically underserved populations.
• Ability to work independently with minimal direct supervision.
• Effective written and oral communication skills.
• Must be organized, self-motivated and have the ability to coordinate multiple tasks simultaneously.
• Ability to exercise sound judgment under crisis situations and to abide by regulations regarding confidentiality.
• Valid NYS Driver's License and ability to drive with current driving record acceptable for agency insurance coverage.
Terms of employment/benefits:
• Non-Exempt.
• Forty (40) hours per week.
