REACH Mission and Values
The REACH Program of Evergreen Treatment Services works with individuals experiencing homelessness and behavioral health conditions to help them achieve stability and improved quality of life. Through outreach, relationships, advocacy and bridging gaps, REACH focuses on reducing harem and supporting healing. REACH provides outreach-based-care coordination, multidisciplinary clinical services, and supports to access and maintain housing. All services are based in principles of harm reduction that offer respect and dignity to individuals moving forward through stages of change in their lives.
REACH incorporates a racial equity lens that includes naming the impact and actively dismantling systems of oppression rooted in White Supremacy, while addressing the root causes perpetuating historical trauma and immense suffering in individuals' lives. We are committed to building a robust behavioral health response that diverts people away from jail by rebuilding community and providing services to ensure those presently marginalized aren't just surviving, but able to thrive.
REACH offers an array of services ranging from survival support provided where folks are living outside to linkages to essential resources such as housing, assistance to resolve legal issues, health care, entitlements and easily assessable treatment for substance use disorders and mental health conditions.

Overview of the LEAD Program
LEAD (Let Everyone Advance with Dignity, formerly know as the Law Enforcement Assisted Diversion program) is a collaborative project aimed at building community capacity to increase public health and safety while decreasing reliance on police and the legal system to respond to public order issues related to drug use and sex work. LEAD provides equity-based solutions for individuals historically overrepresented in the criminal punishment system due to racial criminalization of poverty and homelessness, and the "war on drugs" which has disproportionately harmed communities of color. The LEAD program centers its work in communities existing at the intersectionality of race, identity, intergenerational poverty and chronic drug use and its relationship to White Supremacy. Our work exists in this intersectionality.
REACH LEAD provides comprehensive services to adults experiencing behavioral health challenges to support these individuals to achieve improved quality of life and reduce recidivism. Services include outreach and intensive care management, legal assistance, housing placement, behavioral health treatment and recovery support, and linkage to a variety of community resources. Working with care managers, participants develop an individualized plan based on their needs and priorities. All services are provided from a Harm Reduction framework, utilizing motivational Interviewing skills, and aligned with Trauma Informed Care principles. LEAD partners include community representatives including police and probation prosecutors, and public defenders.
The S/O Coordinator will be responsive to new referrals in the following neighborhoods in West Precinct: Downtown Seattle Business District, Waterfront, International District, Pioneer Square, Belltown, Queen Anne, West Edge, SODO, Westlake, Seattle Center, Denny Triangle, Southlake Union, & Magnolia. The person in this position should have experience working with drug users, unsheltered, individuals engaged in sex work and low-level pimping; engaged and impacted by domestic violence and impacted by the criminal punishment system.
Job Summary
The Screening & Outreach Coordinator will act as the contact point for all referrals to LEAD case management. As such, this position must ensure effective and efficient communication and collaboration between all the partners involved in referring and receiving referrals for LEAD. This position will provide screening, outreach, and engagement to individuals referred to LEAD. They will provide immediate response to Seattle Police Department, King County Sherriff Office and Department Of Corrections officers regarding LEAD participants when LEAD case managers are not available. The Screening & Outreach Coordinator will conduct street outreach as needed to engage referred individuals who have yet to engage with their assigned case manager. The Screening & Outreach Coordinator must develop and maintain positive, collaborative relationships with all LEAD partners and other service providers in order to best serve LEAD participants.
Job Functions:
  • Active point of contact for all referrals to LEAD.
  • Manage number of referrals based on capacity.
  • Complete initial screening with all new referrals.
  • At time of screening assess and address emergent/immediate client needs including need for medical care, shelter, food, and clothing.
  • Provide street outreach to engage individuals who have been referred to LEAD with the purpose of facilitating completion of intake process.
  • Complete client intake as needed.
  • Manage and coordinate the assignment of new referrals to a case manager.
  • Provide case managers with completed paperwork and share additional, relevant information.
  • Ability to track and enter into database all program referrals. Update demographic information, referral date, type of referral, arrest charge and case manager assignment.
  • Develop relationships and liaison with Seattle Police, King County Sheriff's, and other outreach programs to coordinate services.
  • Work in tandem with case managers to provide street outreach to engage participants when needed.
  • Provide immediate response to SPD, KCSO and DOC officers regarding LEAD participants when LEAD case managers are not available.
  • Develop and maintain positive, collaborative relationships with LEAD partners and other service providers including chemical dependency treatment providers, mental health providers, health care providers, shelter providers, landlords, detox centers, DSHS workers.
  • Identify gaps and barriers in available community resources and advocate for systemic changes.
  • Attend all LEAD Program staff meetings and other required meetings.
  • Document all activities according to agency and contract requirements.
  • Additional duties as assigned.

Requirements
Qualification Requirements
Education
  • High school diploma or equivalent required. Undergraduate degree preferred. Relevant experience may substitute for degree.

Experience
  • Demonstrated ability providing street based outreach and engagement services to individuals suspected of low level drug involvement and persons who have experienced trauma and marginalization.
  • Demonstrated experience developing positive, collaborative relationships with law enforcement and social services providers to effectively serve mutual clients.
  • Understanding of substance use disorders and harm reduction strategies along with a demonstrated passion for serving people experiencing homelessness and co-occurring behavioral health conditions required.

Knowledge Requirements
  • Computer literate, with basic knowledge of Microsoft Office Suite, as well as a high level of initiative in keeping current with technological change
  • Ability to prioritize workload and activities of self and complete tasks in a timely and efficient manner
  • Ability to set boundaries, resolve interpersonal conflict and de-escalate issues
  • Dependable, able to work under pressure; receptive to change, willingness to learn, cooperative approach to problem-solving
  • Ability to establish and maintain effective working relationships with staff, clients, and outside contacts from a wide variety of ethnic, socioeconomic and cultural backgrounds; good diplomatic skills.
  • Possession of a valid Washington State Motor Vehicle Operator's License (local travel on behalf of the Agency is required)
  • Must be able to pass a Washington State Patrol criminal background check

Language Skills:
  • Ability to read and interpret general business correspondence, policies and procedures, referral information, financial documentation and applicable government regulations.
  • Ability to write case notes, uncomplicated reports, instructions and procedures.
  • Ability to present information effectively and respond to questions from patients, staff, referral sources and the general public.

Mathematical Skills and Reasoning Ability:
  • Thorough knowledge of and ability to apply business arithmetic skills accurately and rapidly.
  • Ability to solve practical problems and deal with a variety of concrete variables in situations where standardization may be limited. Ability to interpret a variety of instructions furnished in written, oral, schedule or diagram format.
  • Basic math skills

Physical Requirements
  • The employee is regularly required to sit; use hands to finger, handle or feel objects, tools or controls; reach with hands and arms and talk or hear; frequently required to stand, walk and kneel; occasionally to climb balance, or stoop; rarely to crouch or crawl.
  • The employee must occasionally lift and/or move up to 30 pounds. Specific vision abilities required by this job include close, color and peripheral vision and the ability to adjust focus. The noise level in the work environment is moderate.
