Rn Home Health

We are seeking an RN with experience in Home Health to join our team! You will be responsible for the assessments and treatment of our patients at Gordon's Home Healthcare.

Responsibilities:

• Perform assessments (initial assessment, Sup Visits, Reassessments)
• Create Poc per patient.
• Update/ maintain Dmas(97a/b/ 98b , 225 etc forms for care )
• Maintain accurate patient medical records.
• Document and record monthly in-service training

Qualifications:

• Previous experience in nursing or Home Healthcare
• Ability to build rapport with patients.
• Strong problem solving and critical thinking skills.
• Ability to thrive in a fast-paced environment.

Wage

$70 per initial assessment

$40 per Sup Visits

$30 per Reassessment

$60 Monthly Gas Incentive

$23-27 per hour (#10 hours a month in the office)

Open for any negotiation.
