Rn Case Manager Home Health - Hybrid

Job Overview:
We are seeking a compassionate and skilled Registered Nurse to join our Home Health team. As a Registered Nurse - Home Health, you will provide high-quality care to patients in the comfort of their own homes. Duties:
- Perform patient assessments, including vital signs, physical examinations, and medical histories
- Develop and implement individualized care plans for each patient
- Administer medications and treatments as prescribed by physicians
- Provide wound care and perform dressing changes
- Monitor patients' progress and update care plans as needed
- Educate patients and their families on managing their conditions and promoting overall wellness
- Collaborate with interdisciplinary healthcare team members to ensure comprehensive and coordinated care
- Utilize case management skills to coordinate services and resources for patients
- Conduct diagnostic evaluations and utilize critical thinking skills to make informed decisions regarding patient care

Experience:
- Minimum of 2 years of nursing experience, preferably in primary care, ER, or hospital settings
- Experience in Home Health or case management is a plus
- Proficient in patient assessment, including physical examinations and vital sign monitoring
- Strong knowledge of diagnostic evaluation techniques and utilization management principles
- Familiarity with pediatric patient care is desired

Skills:
- Excellent communication and interpersonal skills to effectively interact with patients, families, and healthcare team members
- Ability to work independently while also collaborating with a multidisciplinary team
- Strong critical thinking skills to assess patient needs and develop appropriate care plans
- Proficient in utilizing electronic medical records systems for documentation purposes
