We are currently seeking a Registered Nurse (RN) Case Manager to join our team and play a key role in coordinating and managing the care of our Home Health patients.

Job Description:
As an RN Case Manager in Home Health, you will be responsible for overseeing and coordinating the comprehensive care of our patients. Your primary duties will include conducting initial patient assessments, developing individualized care plans, coordinating services with other healthcare professionals, and ensuring that patient needs are met efficiently and effectively. You will work closely with patients, their families, caregivers, and healthcare providers to ensure continuity of care and optimal patient outcomes.

Responsibilities:

• Conduct initial assessments of patients' health status and develop personalized care plans.
• Coordinate and manage the delivery of Home Health services, including skilled nursing care, therapy services, and medical equipment needs.
• Monitor patients' progress and make necessary adjustments to care plans as needed.
• Collaborate with physicians, therapists, social workers, and other healthcare professionals to ensure coordinated and holistic care.
• Educate patients and their families on health management, medication administration, and self-care techniques.
• Coordinate with insurance providers and ensure timely authorization for services.
• Maintain accurate and thorough documentation of patient care activities and progress.
• Participate in team meetings, case conferences, and quality improvement initiatives.

Qualifications:

• Valid Registered Nurse (RN) license in the state of Illinois.
• Minimum of 2 years of clinical experience in Home Health, case management, or a related field.
• Strong clinical assessment and critical thinking skills.
• Excellent communication, interpersonal, and teamwork abilities.
• Knowledge of Medicare and Managed Care regulations related to Home Health services.
• Proficiency in electronic health record (EHR) systems, i.e Axxess Home Health.

Pay: $64,000.00 - $69,000.00 per year
