We are seeking an experienced RN Case Manager for Home Health to administer skilled nursing care to patients in their home. This is performed in accordance with physician orders and plan of care under the direction and supervision of the Clinical Manager/Nursing Supervisor.
QUALIFICATIONS:
- Graduate of an approved school of professional nursing and currently licensed in IL.
- (2) year of Home Health experience required.
- Minimum of one (3) years experience in nursing.
- Ability to exercise initiative and independent judgment.
- Proficient in Axxess EMR Software
LINE OF AUTHORITY: Reports to Director of Clinical Services
RESPONSIBILITIES:
- Completes and submits OASIS assessments, reassessments, and transfers, resumptions of care, discharges and significant change in condition in accordance with Agency defined time frames.
- Supervises the LPN every 30-days and supervises the CNA every 14-days (or per regulatory requirements) and teaches other nursing personnel.Furnishes those services requiring substantial specialized nursing skill.
- Coordinate with MD and other personnel on changes in the patient’s condition.
- Conducts patient care conferences on patients assigned to his/her care.
- Participates in peer review and Quality Assessment Performance Improvement as assigned.
- Complete documentation and paperwork in a timely manner per Agency policy.
- Demonstrates competent performance of technical skills according to established procedures.
- Demonstrates sound judgment, critical thinking and decision making.
