About the Unit/Department/Team
The RN Case ManagerBehavioral Health works with an interdisciplinary group of providers, nursing staff, patient access and diagnostic service teams.
Mission for Position
The RN Case Manager assesses, plans, implements and evaluates care for patients in the Behavioral Health clinic to ensure each patient has a high quality, personalized experience.
Duties & Responsibilities OR Position Summary
- Assess patient for physical, emotional, social and spiritual needs
- Seeks knowledge and guidance when needed from other team members
- Coordinates patient’s plan of care with patient, family members, physician, interdisciplinary team members and ancillary services.
- Coordinates callbacks, preventative care and medical education for both individual and group sessions.
- Identifies opportunities/needs for patient education
- Advocates for patient
- Documents patient care in EHR
- Evaluates patient’s response to plan of treatment, identifies opportunities for improvement
- Facilitates communication of pertinent information regarding patient care
- Actively involved in quality improvement for department
Position Type
Full time, Monday-Friday, Employee-focused scheduling with hours available between: 8:30a-5pm
Qualifications
Qualifications
- MN RN License
- CPR certification or equivalent
