SalaryHorizon Public Health is seeking applicants for a Full Time Registered Nurse to provide Case Management and Certified Assessor services out of our Elbow Lake office.
A Case Manager provides care coordination, including developing, implementing and evaluating client plan of care, coordinating care using available resources and maintaining contact with care givers. Position will also provide assessment of an individual’s level of care, determination and redetermination of program eligibility, health, psychosocial, functional needs, strengths and preferences, vulnerability issues, environmental needs for safety and access, and need for supports and services.
Minimum qualifications include a Baccalaureate degree in Nursing and licensed in MN with at least one year of home and community-based experience. Exceptions may apply for ADN applicants with at least two years of home and community-based experience. Prior experience in long term care also beneficial.
Successful applicant must pass a criminal background check and a pre-employment drug test. Must have valid MN driver’s license and reliable, insured transportation. This is a Grade 13 MNA/AFSCME Council #65 union position, Step 1 $33.72/hour. Monday-Friday daytime hours.
