Job Summary:
Atrio Home Health is seeking a compassionate and experienced Home Health RN to join our team at Atrio Home Health. The Home Health RN will be responsible for assessing the client's health status, developing and implementing nursing care plans, and maintaining communication with clients, caregivers, interdisciplinary team members, and physicians to ensure that the client achieves the highest level of function possible. The ideal candidate will have excellent interpersonal, analytical, and communication skills and be able to work independently as well as part of a team.

Primary Responsibilities:
● Develop the client’s Plan of Care in consultation with the physician and other care team members.
● Plan for the client’s discharge.
● Provide guidance to assigned staff.
● Prepare documentation according to policy.
● Perform job duties in accordance with the mission and values of Atrio Home Care.
● Understand client rights and incorporate them into daily practices.
● Be compliant with Atrio Home Care and Atrio Home Health’s policy and procedure standards, including HIPAA privacy standards.
● Meet Atrio Home Care and HIPAA standards for confidentiality.
Other Duties & Responsibilities:
● Order medication and supplies related to the plan of care.
● Participate in yearly goal setting for self and departmental development.
● Perform other duties as assigned.
● Be capable and available to work an on-call rotation, which includes evening, night, and weekend coverage.
● Be capable and available to work numerous and successive hours while being oriented (up to 8-10 hours at a time).
Job Qualifications:
● Knowledge normally acquired through completion of an accredited RN program. BSN is preferred.
● Minimum of 2 years of experience in an in-pt. or community mental health setting required for Mental Health Case managers. ANA board certification strongly preferred.
● One to two years of community health and/or home care experience is preferred.
● Licensure as an RN in the State of Michigan is required.
● Reliable means of transportation required.
● Proof of a valid Michigan driver’s license and vehicle liability insurance is required.
● Current CPR certification is required.
● High-speed internet connection highly recommended.
Skills Required:
● Interpersonal skills necessary to communicate effectively with all levels of staff, clients, and their families/caregivers in situations requiring courtesy, tact, respect, and patience.
● Analytical skills to assess client’s health status and to determine appropriate intervention.
● Written and computer skills necessary to maintain required documentation.
● Ability to read, speak, write, and understand the English language.
● Ability to meet Atrio Home Care’s standards for attendance.
● Ability to meet the general health requirements as set forth by the policies of Atrio Home Care, which includes a physical examination, TB testing, and seasonal flu shot.
