PRIMARY RESPONSIBILITY:
- Responsible for providing case management services to children with identified mental health issues in various locations within the community.
ESSENTIAL JOB FUNCTIONS:
- Assess consumer?s needs outside of therapy, completes orientation team meeting (OTM), and completes person-centered plans in coordination with therapist.
- Advocates for consumer and their parent/guardian with external organizations such as schools, other healthcare providers, or community supports.
- Facilitates consumers progress toward treatment goals; provides periodic progress reviews for quality assurance purposes.
- Maintains written documentation of all sessions.
- Utilizes Electronic Charting System to enter all service documentation for appropriate billing and auditing verification.
- Completes regular clinical supervision sessions for case reviews and other administrative problem solving.
- Complies with the Social Work Code of Ethics and the requirements of the Michigan Mental Health Code.
- Complies with agency procedures and policies.
MARGINAL JOB FUNCTIONS:
- Represents the clinic in the community through participation with community service groups or through public presentations
- Participates as a team member on special projects or new offerings.
- Attends administrative staff meetings.
- Contributes to building safety through vigilance and the use of precautions.
QUALIFICATIONS:
- Bachelor?s of Social Work or degree within a related human service field.
- Full or Limited Licensure from the State of Michigan is preferred, but not required.
- Clinical experience with children is desirable.
- Computer and work processing skills are desirable.
- Ability to display reasoning; decision-making skills, and empathy
- Successful completion of Criminal History Background Check is required under the terms of agency employment.
