Job Summary
The Care Coordinator will be responsible for providing a safe and nurturing environment for children and youth between the ages of 6-18. The Care Coordinator will oversee the intake process for all agency programing, including T-Suites, by reviewing referrals and other intake information, conducting interviews, and welcoming clients to the unit. The Care Coordinator serves as a liaison for clients in accessing and coordinating support services.
Essential Job Duties & Responsibilities
  • Welcome children entering the facility upon entry and admission.
  • Coordinates services with other agency programs and staff.
  • Collects reviews and evaluates referral material and client information.
  • Interviews clients and prepares intake documents, as necessary.
  • Interviews placement agency workers.
    • Interview other personnel associated with the placement process
    • Requests additional information from referring agencies, as necessary.
  • Coordinates all youth placements with program staff.
  • Maintains intake documents and logs of all referrals.
  • Maintain case records and documentation in compliance with regulations and standards;
    • Completes necessary paperwork.
  • Provides care, support, attention, and a positive, therapeutic atmosphere to all children in the department to which assigned.
    • Maintain consistent, firm, and fair relationships with all children.
    • Serves as an appropriate, positive role-model
  • Attends and participates in all meetings, staffing's, SARS and court hearings related to assigned clients.
    • Works cooperatively with staff as a team member in assessing client's adjustments and total program effectiveness
  • Provides physical and emotional security for the children through intervention, correction, and re-direction of negative behavior when:
    • Behavior is damaging to self;
    • Behavior is damaging to others
    • Behavior may result in damage to property
  • Assists with managing the physical needs of the children including health, hygiene, and physical comfort (food/clothing/shelter)
  • Educating clients and families about community resources to help address any social determinant of health needs.
  • Provides formal and informal reports on behavior, attitude, and general disposition of assigned children, including Incident Reports, restraint remediation sheets, time-out reports; Department logs; Behavior Rating Scales; Primary sheets
  • Attends conferences and meetings.
  • Participates in the planning, supervision, and provision of therapeutic and recreational activities
  • Transports children as necessary including, but not limited to medical appointments, activities, school
  • Assist with serving as a community resource for local agencies and others interested in the program.

Job Qualifications
  • Bachelor's degree in Social Work or related field. A Master's degree in Social Work or related field preferred.
  • A minimum of two (2) years' experience in counseling, childcare, social work, and/or in providing therapeutic support services to children and families; and/or
  • An equivalent combination of education, training, and experience.
  • A valid Driver's License, appropriate driving record with appropriate insurance coverage (as required by agency policy and/or insurance carriers)
  • Licensed by the State of Ohio Counselor and Social Work Board as a Licensed Social Worker (LSW), or a Professional Counselor (PC) preferred.

Physical Demands/Work Environment
This job operates in a residential childcare environment, which includes crisis intervention, and de-escalation. This role routinely uses standard office equipment. While performing the duties of this job, the employee is regularly required to speak and hear. The employee is frequently required to sit for extended periods of time, stand, walk, use hands and fingers, and reach with hands and arms. Travel and work outside of normal hours may be required for meetings, presentations, trainings, and other events.
