Home Health Admissions Nurse (RN)
1) Initial admission of Home Health patients and preparing patients to be handed off to case managers.
2) Communication with the physician who is responsible for the Home Health plan of care related to the current Home Health plan of care.
3) Coordination and assigned of care to team members and resources
4) Routinely complete Start of Care/Resumption of Care/Recertification/Discharge of OASIS visits and may occasionally need to provide routine visits based on individual patient needs and branch expectations
5) Ensures plan of care incorporates and guides appropriate teaching related to health maintenance, prevention and safety
6) Coordinates available resources to manage care plan and ensures stated outcomes are achieved
7) Periodically reassesses or delegates the reassessment of patient needs and revises care plan as necessary
8) Assures appropriate care of patient is met through the start of care assessment completion
9) Promotes/coordinates communication between team members, attending physicians, appropriate administrative staff, referral sources and external case managers to ensure appropriateness of care
10) coordination and communicates any necessary changes to the plan of care
11) Ensures patient needs are identified and qualified caregivers are assigned to all cases within their scope of practice
12) Participates in special projects and performs other duties as assigned
*** AXXESS EMR EXPERIENCE IS A RECOMMENDED ***** HOME HEALTH EXPERIENCE ******
Position Qualifications
- Current, unencumbered AZ RN license
- 2 years RN experience required.
- Minimum of 1 year Home Health RN experience preferred.
- AXXESS experience preferred.
- Valid driver's license with proof of insurance in accordance with state requirements
- Able to educate patients and family on multiple disease processes, has good medication knowledge and wound care experience.
