AREA COVERAGE:
- East Valley: Phoenix, Apache Junction, Chandler, Gilbert, Mesa, Tempe, Queen Creek
SCHEDULE:
- 40 hours a week for 5 days
- 8:00 AM to 5:00 PM
ABOUT THE POSITION:
The Registered Nurse is responsible for traveling to a patient’s home to administer their services and help patients maintain their independence. Their duties include administering at-home IVs, changing dressings, cleaning wounds, and updating Doctors about their patient’s health.
- Initiates the plan of care and necessary revisions including obtaining written physician orders for changes
- Administer initial, ongoing comprehensive assessments, chart observations, and follow-up skilled nursing visits in the patient's home.
- Coordinate appropriate care, encompassing various healthcare personnel (such as Physician, Physical Therapists, Occupational Therapists, Home Health Aides, and external providers) that are in the patient’s care plan
- Effectively plans and organizes work assignments, manages and sets competitive priorities, and completes work assignments with a minimum of direct supervision and within established time frames.
- Perform duties consistent with the Nursing Practice Act including the Standards of Competent Performance and adhere to the company policies and protocols.
- Attends care conferences, initiates interdisciplinary referrals, and participates in care coordination as needed.
Requirements:
- RN License in the State of Arizona or with Multistate
- AZ Fingerprint clearance card
- Updated Credentials:
- CPR / BLS
- Negative TB Test Result
- Driver's License and Auto Insurance
- Flu Shot / Signed Declination Form
- Hep B / Signed Declination Form
- COVID-19 Vaccination / Signed Religious Exemption Form
- Immunization Records (MMR, Tdap, Varicella)
Others:
- Will be driving to the patient's residences
- Must be able to lift 25 lbs
- Must be in good health and be able to fulfill the physical requirement
- Must be willing to drive all over East Valley
- With Home Health Care experience preferred
- W4 type of employment
