Referral and Support Specialist

HOURS:
40 hours per week, flexible. Must be able to work flexible and/or additional hours as mandated by Board operational needs. Hybrid Work Environment.
JOB SUMMARY:
This position is responsible to support individuals and families who are not currently enrolled on a Medicaid waiver and do not require any type of service authorization, by maintaining communication, providing information, making community connections, and administering the Family Engagement Program. Must strive to meet the Agency mission and exemplify the Agency’s core values.
JOB REQUIREMENTS AND EXPECTATIONS
Minimum Qualifications:
Education: Associate’s Degree in Social Work or a human service related field required.
Experience: One (1) year of verifiable paid or educational field experience working with individuals with developmental disabilities.
Skills: Proficient in use of Microsoft Office. Operate job related equipment including the use of basic computer programs and systems. Read, understand, and communicate information and ideas presented in writing. Use mathematics to solve problems.
Certifications: DODD Service and Support Administrator
KNOWLEDGE, ABILITIES AND OTHER EXPECTATIONS
Knowledge: Administrative and clerical procedures and systems such as word processing, managing files and records, preparing meeting minutes, designing forms, and other office procedures and terminology. General principles and objectives of information technology systems and programs. Proper spelling, grammar, punctuation, and sentence structure to ensure that written materials prepared and reviewed are complete, succinct, and free of errors.
Abilities: Do data entry, generate reports, copy, manage and track documents and information. Effectively manage time to meet operational needs and desired outcomes and plan and organize the steps to implement new projects. Listen to, understand and communicate information and ideas. Work independently with minimal supervision. Maintain strict confidentiality. Work as part of a team and work effectively with persons and communities from diverse cultural, social, and ethnic backgrounds. Work effectively under pressure.
Other Expectations: Report unusual incidents and major unusual incidents. Complete all required training and maintains strict confidentiality. Regular attendance is mandatory. Maintain related records in accordance with appropriate records retention guidelines and policies. Adhere to health and safety policies and procedures. Must have reliable transportation to fulfill responsibilities requiring travel in the community. Must be available by personal cell phone during working hours.
WORK ENVIRONMENT: OUTDOOR / COMMUNITY
This is a community-based role. Work is normally performed primarily in the community. The employee may be subject to extreme and adverse weather conditions. May be exposed to blood-borne pathogens, communicable diseases, potentially infectious materials and/or aggressive behavior. May be required to travel. May be exposed to fumes/odors; work with lubricants, chemicals, and cleaning solvents.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Evaluation of this position is based primarily on performance of the following essential functions, which include, but are not limited to:
For individual and families without any direct service needs:
1. Maintains routine communication with assigned individuals and families.
2. Documents relevant information in the Individual Service Plan and when appropriate, develops individualized outcomes.
3. Provides information, connects to community resources, and makes needed referrals.
4. Attends meetings or community activities with individuals and/or families.
5. Administers the local Family Engagement Program according to program procedures.
6. Gathers, maintains, and shares information about community resources and events.
7. Recognizes service gaps in the system and works with agency and community partners to develop solutions.
8. Assists individuals and families in navigating systems, including the school system.
9. Develops avenues to keep families connected to Summit DD, especially during elementary, middle and high school.
10. Links individuals and families to a SSA when necessary and maintains a collaborative relationship with the team.
11. Assist individuals, families and community partners in linking to Summit DD services.
12. Must be able to work collaboratively and effectively with the administration, other agencies, professionals, staff, families, and the public.
13. In accordance with DODD requirements, will be responsible for documenting and submitting Medicaid billing documentation in accordance with the Targeted Case Management and / or Medicaid Administrative Claiming Guide.
14. Attends all required training; identifies any training needs, and requests training and Support.
15.Performs other related duties as assigned.
Job Type: Full-time
Pay: $17.50 - $20.61 per hour
