FLSA Status: Non-Exempt
Salary: $18.00/hour with an additional $1.00 shift differential on weekends.

About the Role
As a member of a self-directed team, the Integrated Support Specialist will work collaboratively and independently to set and achieve outcomes for the individual assigned. This position is responsible for ensuring the services and support of individuals served based on the goals set in their ISP and the needs of the program. Provides either directly or indirectly the provision and coordination of a variety of supports for people receiving services that will ensure their health and safety.
Essential Functions of the Role
- Work closely with other members of the self-directed team to develop workflow, assignments, and schedule
- Assist individuals served to coordinate services including employment, community access, activities, finances, health, and wellness
- Coordinates with Technology-Enabled Support (TES) team to ensure individuals' needs are met for all services
- Collaborates with other members of the self-directed team to set and achieve outcomes and track progress toward those outcomes .
- Develop and implement targeted intervention services based on individual’s needs
- Represent LADD in the Individual Service Plan (ISP) for each person receiving services on the assigned caseload and review completed plans for health/safety needs, accuracy, and consistency with LADD’s documentation system.
