Brief Description
ABOUT THE JOB
This position will work with the Public Health clinical team that provides a variety of services to the community to include Family Planning services, communicable disease investigation, Tuberculosis case management, Home Visiting programs, and immunizations.
This position is responsible for investigating reportable communicable diseases through telephone case interviews, contact tracing; act as a consultant for outbreaks in congregate facilities; provide education to community members & providers regarding communicable diseases. As well as, utilizing standing orders and protocols to provide birth control options, STI screening, treatment, and education for clients, utilizing standing orders to provide immunizations to clients; coordinating yearly outreach immunization events in the community, and providing case management, education, and support to community members diagnosed with active Tuberculosis.
BONUS ELIGIBLE
*This position is eligible for a Commitment Bonus equivalent to 3 months salary with an agreement to stay with Polk County Public Health for a period of at least 24 months
MINIMUM QUALIFICATIONS
Graduation from an accredited school of nursing with an associate's degree in nursing, BSN Preferred. Experience in Public Health, hospital or a clinic setting is preferred, but not required.
SPECIAL QUALIFICATIONS
Must possess and maintain a license to practice as a Registered Professional Nurse in the State of Oregon at the time of appointment.

Our Community:
Polk County is located in the Willamette Valley just an hour from the Pacific Coast to the west and an hour from Portland to the north. The population is approximately 87,000 and the county spans about 744 square miles, with two thirds of the western part of the county being forest. National protected areas include the Baskett Slough National Wildlife Refuge and Siuslaw National Forest.

Benefits Overview:

Paid Leave:
  • Vacation: New full-time employees generally earn 12 vacation days per year; this increases with years of service. Part-time employees earn a pro-rated amount.
  • Holidays: A generous number of paid holidays annually. This will vary according to bargaining unit.
  • Personal Leave Days: These are floating holidays. The number of Personal Leave Days varies according to bargaining unit.
  • Sick Leave: 8 hours per month (pro-rated for part time) with no maximum accrual.
  • Other Leaves: Jury duty, bereavement, military.
