The department offers services at more than 40 locations in Deschutes County including public schools; health clinics in Bend, La Pine, Redmond and Sisters; five school-based health clinics; agencies such as the KIDS Center and the State of Oregon Department of Human Services; area hospitals; care facilities and homes.

ABOUT THE JOB:

The Public Health Nurse II, Family Support Services provides personal nurse visits for clients that Include comprehensive maternal and infant assessments, case management, care coordination, linkage to community resources, and parent skills building and education. Programs include CaCoon (case management for children with special health care needs), Babies First (prenatal, postpartum and 0-5yrs services) and Family Connects (universally offered postpartum visiting). This position will specifically provide Babies First and CaCoon services in Deschutes County as both are Evidence informed programs supported by Oregon Health Authority.
This position requires travel throughout Deschutes County to include Bend, Redmond, Sisters and La Pine. Cross-training in other nurse home visiting programs may be required as appropriate based on department needs.
Key Responsibilities:
  • Complete comprehensive nursing assessments via in-person and telehealth to evaluate the health status of prenatal and postpartum caregivers and children 0-5 rears old, and CaCoon eligible clients 0-21 years old..
  • Document visit encounters to include nursing assessment, nursing diagnosis, planning, implementation, and evaluation of care in our EHR and data collecting systems.
  • Provide prenatal, postpartum and pediatric education.
  • Provide case management, care coordination, and refer families to community resources.

  • Working with high-risk clients in the maternal child health nursing field.

  • Charting in electronic health record systems.
  • Working independently, being self-motivated and resourceful, and having the ability to keep organized with good time management skills while managing a full caseload.

  • Associate's degree in Nursing;
  • AND two (2) years of relevant experience;
  • OR any equivalent combination of training, education, and experience that provides the required skills and knowledge to perform the essential functions of the job.

Preferred Qualifications:

  • Associate's degree in nursing and four (4) years of experience in Public Health nursing or related nursing experience in public or community health.
  • A Bachelor's degree in nursing with two (2) years of Public Health or maternal child health nursing experience.
  • Bilingual in English and Spanish.

Driving is a requirement for this position. Possession of or ability to obtain a valid Oregon driver's license within 30 days of hire date. The employment offer will be contingent upon presentation of an acceptable and verifiable driver's license, pre-employment screening for criminal history, driving history, and controlled substances (NOTE: Positive test results for marijuana use may result in rescission of a contingent offer of employment). This screening must be completed with satisfactory findings in order for a formal offer of employment to be extended.

Must have dependable transportation and be able and willing to drive throughout Deschutes County in sometimes adverse weather conditions.

Possession of or ability to obtain an Oregon State Registered Nurse License by time of hire.
Sufficient ambulatory ability to deliver services throughout Deschutes County, including the materials needed for presentations, and to provide face-to-face services in a variety of settings in the community, including but not limited to hospitals, schools, jails, clinics, client homes, and community and residential settings within required response times.
Physical Requirements. Lift and carry equipment up to, but not exceeding 30 pounds. Climb stairs, stand for extended periods, walk distances of 100 feet, and sit down on the floor and get back up without assistance.Employment Type: Regular, full time, full benefits
