P/T Outreach Specialist

We are seeking an enthusiastic and motivated Outreach Specialist to join our team. As an Outreach Specialist, you will play a crucial role in promoting our organization and engaging with the community. You will be responsible for developing and implementing outreach strategies to increase awareness and participation in our programs and services.

Responsible for conducting testing outside the office and targeted community outreach and engagement, risk reduction education, referrals, and case documentation including appropriate follow up based on acuity levels

• Seeks referrals from primary care medical offices for patients diagnosed with HIV or other infectious diseases.
• Promotes Midway Specialty Care Center, Inc., to primary care medical offices and other agencies.
• Performs administrative support in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.
• Communicates appropriately and clearly to co-workers, providers, physicians and patients.
• Meets current documentation standards and policies.
• Maintains established office policies and procedures, objectives, performance improvement program, safety, environmental and infection prevention and control standards.
• Answers telephone calls in a professional manner, directs calls appropriately; refers patient problems to office physician; schedules patient appointments.
• Maintain consistent on-going communication with medical providers to streamline the linkage to medical care process and obtain verbal and written verification of services obtained by clients referred.
• Maintains a good working relationship with all staff members of the office.
• Maintains patient confidentiality at all times.
• Reports to work on time and as scheduled; completes work within designated time.
• Wears identification while on duty, uses computerized time clock system correctly and consistently.
• Represents the organization in a positive and professional manner in the community.
• Complies with all organizational policies regarding ethical business practices.
• Communicates the mission, ethics and goals of the office.
• Completes face to face needs assessments of individuals.
• Establish treatment programs by setting schedules and routines; arranging resources, including transportation and escort.
• Works to ensure patients are enrolled in an insurance program or other program to cover medical expenses.
• Use e ClinicalWorks for notes and evaluations (as well as paper file if desired).
• Perform all other job related tasks as asked.

Required Skills & Abilities:

• Knowledge and understanding of HIPAA regulations
• Basic knowledge of HIV/AIDS treatment and care.
• Maintains knowledge of pertinent laws and regulations regarding HIV testing and counseling as well as professional principles and practices to the fields of health care and health date analysis.
• Ability to be flexible, organized and function under stressful situations. Response to the patient is appropriate in emergency or physically distressful situations.
• Experience in HIV testing, counseling and HIV data dissemination.
• Demonstrate proficiency in social media marketing.
• Ability to work with considerable independence.
• Ability to demonstrate cultural humility.

Language Skills:

• Able to communicate effectively in English, both verbally and in writing.
• Additional languages preferred.
• Basic computer knowledge.

Qualifications:

• High school diploma or equivalent required; Associate’s degree in office administration or related field preferred. High school diploma or equivalent required.
• Must be an HIV Certified Tester in the State of Florida – 500-501 Certified or have the ability to become certified within 90 days of employment.
• Valid Florida Driver License.
• Reliable transportation.

Pay: $18.00 - $20.00 per hour

Expected hours: 20 – 30 per week
