Overview:
We are seeking a compassionate and skilled Part-Time Registered Nurse (RN) to join our team. As a part of the Senior Helpers team, you will play a vital role in assessing needs, developing and implementing care plans, and coordinating services to ensure our clients receive the best possible care in their homes. This position will be roughly 16 hours a week and offers a flexible schedule between the hours of 9am-5pm.
Responsibilities:
- Conduct comprehensive nursing assessments on new and existing clients.
- Develop and implement individualized care plans in collaboration with Care Manager and Client Service Manager
- Educate clients and their families on disease management and home safety measures.
- Document all care provided in a clear and concise manner according to agency and regulatory standards.
- Maintain strong communication with Care team, families, and other healthcare professionals involved in the client's care.
- Work collaboratively with the Home Care team to ensure seamless coordination of services.
- Adhere to all company policies and procedures, including infection control standards.
- Provide nonskilled care to clients
Qualifications:
- Active RN license in the state of Wisconsin
- Minimum of 4 years of experience in Home Care or a related setting.
- Experience working with VA and/or Medicaid
- Strong clinical skills and assessment abilities.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Time management and organizational skills.
- Valid driver's license and reliable transportation.
