PRIMARY FUNCTION
The Psychiatric Care Coordinator supports the medical and behavioral health needs of patients who are receiving treatment from a psychiatric provider at White House Clinics. The Psychiatric Care Coordinator reports to the Behavioral Health Program Manager and works directly with the psychiatric provider to facilitate the care needs of patients receiving Psychiatric Care. The Psychiatric Care Coordinator is responsible for managing patient flow, addressing scheduling needs, and serving as an intermediary between the psychiatric provider and the medical and behavioral health providers.
PRINCIPLE DUTIES
  1. Participates in a morning huddle with the psychiatric provider at the start of each day, reviewing patients on the schedule and identifying any specific care needs. Responsible for addressing these needs throughout the day as appropriate (i.e. orders for lab work, prior authorizations, additional testing or screening, etc).
  1. Rooms patients for visits with psychiatric provider, following relevant intake protocols for taking a patient history, recording vital signs, medication reconciliation, pre-visit screenings and other components of intake relevant to Psychiatric Care.
  1. Coordinates with patient access staff to facilitate efficient rooming of patients, both for patients presenting in the clinic and patients being seen by telehealth.
  1. Contacts patients after missed appointments with the psychiatric provider to identify care needs and coordinate rescheduling.
  1. Responsible for submission of prior authorization requests for visits and/or medications as indicated. Follows up on prior authorization requests until prior authorization process is complete.
  1. Works with psychiatric provider to review patient referrals to determine appropriateness of referral and assists in scheduling these patients.
  1. Collaborates with medical and behavioral health providers to provide wraparound care coordination, including activities such as:
  1. Following up with patients who have missed appointments
  2. Facilitating collaboration between psychiatric provider and PCP
  3. Coordinating referrals for patients to outside providers, ensuring successful completion of the referral
  4. Collaborating with Enabling Services and Patient Financial Services staff to remove barriers to care
  1. Maintains a current resource list of Behavioral Health and substance use disorder community resources available to patients (i.e. specialty behavioral health assessment and counseling services, case management resources, therapeutic groups, etc.) and makes this information accessible to all provider staff.
  1. Assists in referring patients to outside behavioral health resources when needed.
    1. Discusses referral options with patient.
    2. Facilitates initial contact with outside service.
    3. Obtains appropriate releases of information to exchange behavioral health information.
    4. Follows up with outside service to confirm patient connection with the resource.
  1. Assists Medical Records staff in obtaining inpatient behavioral health treatment records upon patient discharge.
  1. Participates in relevant quality improvement projects related to the care of patients with behavioral health and Psychiatric Care needs.
  1. Employees are responsible for maintaining a thorough understanding of their role in the following protocols and as protocols are updated periodically employees are expected to remain current on any updates. As the position develops, employee is responsible for remaining current on any new or updated protocols relevant to the position.

  1. Assists with other duties or in other departments as instructed by supervisor.
  2. JOB REQUIREMENTS
Minimum Education
High School Diploma or GED.
Minimum Work Experience
One year of experience working in a medical office or community-based organization.

Provide superior customer service to all patients, external customers (office staff of other offices, hospital, community agencies, etc.), and co-workers. Serves as an ambassador of the White House Clinics in all conversations and interactions with these parties.

Maintains clinic standards for a clean and quiet patient environment to maintain a positive patient care experience. Promotes culture of safety for patients and employees through proper identification, reporting, documentation, and prevention.

Maintains compliance with organization’s policies, as well as established practices, protocols, and procedures of the position, department, and applicable professional standards. Complies with organizational and regulatory policies for handling confidential patient information. Adheres to professional standards, clinic policies and procedures (OSHA, HIPAA, Standing Orders, etc.), federal, state, and local requirements, Health Resources and Services Administration (HRSA), U.S. Department of Health and Human Services (HHS), Office of the Inspector General (OIG), etc. Cooperates fully with all aspects of Corporate Compliance Plan and Standards of Conduct. Participates in all safety programs which may include assignment to an emergency response team.

TYPICAL PHYSICAL DEMANDS
This position requires sitting, some bending, stooping and stretching. Eye-hand coordination and manual dexterity sufficient to operate a keyboard, photocopier, telephone, calculator, and other office equipment are also required. Position requires normal range of hearing and eyesight to record, prepare, and communicate appropriate reports. Employees will be required lift papers or boxes up to 50 pounds occasionally.
TYPICAL WORKING CONDITIONS
Work is performed in office environment and involves frequent contact with staff and the public. Position may involve dealing with angry or upset people. Evening and/or weekend work is required. Work may be stressful at times.
SALARY
$15.04 - $23.68
WORK HOURS
37.5 hours (Irregular work hours, some overtime possible at times.) EVALUATION
Evaluated annually by the Behavioral Health Program Manager and providers as appropriate.
