Patient Care Coordinator

Horizon Adult Health Care in Irvine, KY, an EOE, has the following job opportunity available: Patient Care Coordinator

The position is full time, daytime hours, Monday-Friday. The position will require public education about services offered and assistance with applying for services. Experience working with the geriatric/disabled population preferred, but not required. The position will require travel within county. During extenuating circumstances such as pandemics, job duties may transition to include providing additional services in client homes.
