This is a new position at Gateway To Success that is a full-time GRANT funded position coordinating FACT services in Pueblo (.5) and Canon City (.5).
Forensic assertive community treatment (FACT) is a service delivery model intended for individuals with serious mental illness (SMI) who are involved with the criminal justice system. These individuals may have co-occurring substance use and physical health disorders. Their needs are often complex, and their disorders are often under-managed and further complicated by varying degrees of involvement with the criminal justice system. FACT builds on the evidence-based assertive community treatment (ACT) model by making adaptations based on criminal justice issues—in particular, addressing criminogenic risks and needs. In this sense, FACT is an intervention that bridges the behavioral health and criminal justice systems. FACT is designed to do the following: improve clients’ mental health outcomes and daily functioning; reduce recidivism by addressing criminogenic risks and needs;1 divert individuals in need of treatment away from the criminal justice system; manage costs by reducing reoccurring arrest, incarceration, and hospitalization; and increase public safety.
The objective of the FACT Grant is to increase the stabilization of individuals suffering from persistent severe mental illness with criminal justice involvement who may also have co-occurring or physical health disorder by offering high intensity behavioral health treatment to reduce recidivism and relapse by at least 25%; therefore, increasing stabilization and quality of life for the clients we serve.
The Program Coordinator will work directly with the Program Manager and assist in developing the program. They will over see the program as a whole and reach out to referral sources and make sure everyone is on track to achieve our grant goals.
The Program Coordinator will be responsible for developing and implementing processes, under the guidance of the Project Manager, that ensures seamless access to care for identified clients. Ensure access to behavioral health professionals to address mental health, substance use, co-occurring disorders and promote emotional well-being. This will be conducted by ensuring clients are scheduled and complete a mental health evaluation and treatment plan upon entry into services and are engaging in services.
The Program Coordinator will participate in Multi-Disciplinary Team Meetings and maintain accurate records for reporting to both the referral sources and grant funders.
Schedule: Monday- Friday (Hybrid Scheduling)
Compensation: $60,000 per year/Salary
