We are looking for a caring and compassionate, PRN Home Health PT (or Physical Therapist). We are looking for someone who can for our hospice patients in King County, Kitsap County, and Pierce County.

Job Summary:
The Registered Physical Therapist (PT or RPT) is responsible for administering physical therapy to patients on an intermittent basis in their place of residence. This is performed in accordance with physician orders and plans of care under the direction and supervision of the Director of Clinical Services/Nursing Supervisor.
Responsibilities:
- Provides physician-prescribed physical therapy takes initial history and makes an initial evaluation.
- Directs and supervises personnel as required.
- Participates with all other health care personnel in patient care planning.
- Consults with physicians regarding changes in treatment and reports to physicians regarding the patient's progress.
- Instruct patients and family/significant others in-home programs and activities of daily living.
- Participates in in-service programs and presents in-service programs as assigned, participates in performance improvement activities as assigned.
- Attends all patient care conferences as scheduled.
- Consult with family and Envision personnel.
- Prepares clinical and progress notes and completes and submits OASIS assessments, reassessments, transfers, resumptions of care, discharges, and significant changes in condition in accordance with the Envision's defined time.
- Appropriately utilizes ICD-9 codes.
Qualifications:
- Graduate from a Physical Therapist curriculum approved by the American Physical Therapy.
- Must have a Physical Therapy License from the state of Washington.
- Medical Association and American Physical Therapy Association.
- Must have at least one (1) year of experience as a licensed Physical Therapist.
- Ability to exercise initiative and independent judgment.
