TITLE OF POSITION: PERSONAL CARE SERVICE PROVIDER
The Personal Care Service Provider will provide non-medical hands-on assistance to clients to assist with activities of daily living such as grooming, dressing, bathing, toileting, eating, getting into or out of a chair, walking, or reminding clients to take medications.
DUTIES
- Provides Personal Care and related services in the home, under the direction, instruction, and supervision of the Director of Client Services
RESPONSIBILITIES
- Follows the service plan to provide, safe, competent care to the client
- Helps the client to maintain good personal hygiene and assists in maintaining a healthful, safe environment
- Plans and prepares nutritious meals, markets when instructed to do so by the service plan
- Assists the client with ambulation as directed by the service plan
- Encourages the client to be as independent as possible according to the service plan
- Attempts to promote client's mental alertness through involvement in activities of interest
- Gives simple emotional and psychological support to the client and other members of the household and establishes a relationship with client and family which transmit trust and confidentiality
- Reports any change in the client's mental or physical condition or in the home situation to the staff nurse, or to the Director of Client Services
- Performs routine housekeeping tasks as related to a safe and comfortable environment for the client, as instructed by the service plan
- Prepares a visit report/note promptly and incorporates same in the clinical record weekly
- Confirms on a weekly basis, the scheduling of visits so that the client is informed
- Works with personnel of other community agencies involved in the client's care as directed by the service plan of Director of Client Services
- Attends in-service as required by licensure, regulation, and agency policy
- Reminds the client to take medications
JOB CONDITIONS
- The ability to drive and the ability to access clients’ homes which may not be routinely wheelchair accessible are required.
- Hearing, eyesight, and physical dexterity must be sufficient to perform a physical assessment of the client's condition and to perform client care.
- On occasion, may be required to bend, stoop, reach and move client weight up to 250 pounds; lift and/or carry up to 30 pounds.
- Must be able to communicate clearly, both verbally and in writing.

QUALIFICATIONS
- Be 18 years old and preferably a high school diploma or equivalent
- Must have completed a standardized New Hampshire state LNA training program and the LNA standardized LNA competency test
- Must be free from health problems that may be injurious to client, self and co-workers and must present appropriate evidence to substantiate this
- Must understand and respect client’s including ethics and confidentiality of care
- Must hold a current CPR certification
- Must have a complete Criminal Background and BEAS check per state and federal mandates.
Job Types: Full-time, Part-time
Pay: Up to $23.00 per hour
Expected hours: 15 – 40 per week
