Location: 6701 Peters Creek Rd, Roanoke, VA 24019

Hours: Monday-Friday, 8:00am-5:00pm


Enhabit Home Health & Hospice is hiring a Patient Services Coordinator (Scheduler) to join our team! The Patient Services Coordinator will schedule patients for field clinicians. They will communicate with field staff, patients, physicians, referral sources, caregivers, and other service providers to maintain proper care coordination and continuity of care. This position will also manage the on-call notebook and hospitalization logs to enhance communication among stakeholders.

  • Scheduling home visits for the field staff each day and assisting with the coordination of patient care.
  • Manage and complete patient schedules; assign visits timely to ensure completion of patient care, compliance with orders, and allowance for staff to appropriately prioritize visits daily.
  • Ensuring geographic scheduling to reduce drive time and to enhance staff satisfaction.
  • Accumulating admissions and intake information.
  • Communicating with physicians about orders and patient progress information.

  • Must possess a high school diploma or equivalent.
  • Must meet one of the following requirements:
    • Be an LPN/LVN licensed in the state in which he/she practices, with at least one year of clinical experience in a healthcare setting.
    • Have at least one year of home health or hospice experience within the last 24 months and have a demonstrated understanding of staffing and scheduling requirements related to home care services.
  • Must have basic demonstrated technology skills, including operation of a mobile device.
  • Previous experience in home health, hospice, or pediatrics is preferred.
  • Must possess a passion for helping patients.
