Patient Care Coordinator/Scheduler

Job description

As a Patient Care Coordinator you’ll be responsible for collecting key clinical information from both patients and providers. Schedule out Specialty medication deliveries. Heavy attention to detail. Fast and very busy environment. Must posses' a positive and teamwork mentality. Reliable is a must.

Essential Functions:

• Effectively communicates with patients, their families and or support systems, and collaborates with physicians and ancillary service providers to coordinate administration of medications. Obtains complete physician orders and referral information to the nurse agencies as needed.

Skills:

• Experience with scheduling in the medical field is a plus
• Professional approach with all referral sources.
• Communication skills, written, verbal, spoken with a broad spectrum of individuals.
• Attention to detail, along with excellent customer service
• Proficient computer Skills
• Organizational skills to include prioritization, planning, implementing, and controlling. Demonstrate necessary skills and knowledge to coordinate work flow
• Must have excellent interpersonal skills
• Maintains respect for the dignity of every person by addressing issues and concerns with workers directly, with a positive problem-solving approach, and the observance of the right to patient privacy and confidentiality.
• Demonstrates concern, respect, and caring for all customers, both internal and external, regardless of their diagnosis or socioeconomic status.
• Maintains positive interpersonal relations.
• Performs other related job duties as assigned.

6 months min. experience in Home Health/Medical Office setting preferred

6 month min. Scheduling experience Preferred (Medical Assistants or LVN's are encouraged to apply)

Pay: Start: $17.00-$20.00hr DOE

+Monthly bonus after probation period
