Job Summary:

The official job title is Lead Care Manager (LCM). As an LCM, you will play a pivotal role in ensuring the holistic well-being of patients by identifying their medical and social service needs, educating them about their conditions and medications, and developing tailored care plans. Your responsibilities will extend to responding to patient inquiries and concerns, maintaining compliance with hospital and legal requirements, collaborating within interdisciplinary teams, and facilitating the coordination of medical and social services.

Responsibilities:

  • Interview patients to identify their medical and social service needs.
  • Educate patients about their condition, medication, and give specific instructions.
  • Develop a care plan to address their personal health care needs.
  • Respond to patient's questions and expressions of concern.
  • Comply with hospital and legal requirements by fostering nursing practices that adhere to the hospitals and nursing division's philosophy, goals, and standards of care; requiring adherence to nurse practice act and other governing regulations.
  • Work as part of a team with other members of the interdisciplinary team.
  • Locate and contact providers of medical care.
  • Facilitate the coordination of social services for patients.
  • Perform other duties as assigned.

Requirements:

  • Medical terminology or background in healthcare
  • Experience in care coordination, case management, or clinical practices
  • Strong analytical thinking and ability to handle multiple tasks concurrently
  • Must have a valid California driver's license and valid vehicle insurance.

Qualifications:

  • High school or equivalent
  • Medical terminology: 1 year
  • Patient care: 1 year (Preferred)
  • Case management: 1 year (Preferred)
  • Medical documentation: 1 year (Preferred)


Pay range

$24—$27 USD
